I need to upgrade the contact form that I deploy across my websites. The contact form can be made with html or php but preferably without a database. The form should have it's own css file, config file and OTP. The form should contain the following fields: Name:* Email Address:* Address 1:* Address 2: Town:* Post Code:* Mobile Telephone Number: Landline Number: (only appears if a mobile number is not provided) One Time Verification:* (drop down list offering choice of text or email) Choice of Reply:* (drop down list offering choice of email or telephone) Recipient:* (in the form of a drop down list specified in the config file with email) Topic:* (in the form of a drop down list specified in the config file) Message: (unlimited plain text) [Clear Button] [Send Button] * Mandatory * Country: (drop down list for country selection) On clicking send you will receive an OTP (one time code) that you need to enter into the boxes below in order to verify your choice of mobile telephone number or email address. Please note that the OTP expires in 20 minutes. [6 boxes for OTP verification] [Verify Button after code is entered] End of form. When completed and verified the form needs to be sent to the person and email address as selected in the config file but senders should not see the recipients email address only their name. The successful freelancer will be responsible for ALL testing with domain based email accounts as well as ALL free services. The process should not be affected by any security protocols in place not limited to DMARC, DKIM, SPF, TLS, S/MIME, PGP. If you have any questions please ask them BEFORE you submit a quote. To demonstrate that you have read and understood the requirements please include the phrase "I have read and fully understand your requirements" at the top of your quote.
