443489 Contact forms

I need a series of SIMPLE PHP scripts where people can leave certain information. Think in terms of a couple of text boxes, a couple of drop boxes and a picture for each form. They need to be able to check the form before they send it. And the form needs to save it in a mysql. I also need a SMALL and SIMPLE admin part to see all the results for every form. I need 8 forms. I will provide the information needed for each from. Most of it, though, is the same.
