Recording working hours

We need to record working hours for our staff on the Railway infrastructure as part of our compliance. I need a spreadsheet drawn up which records these. The first pic ("data input") is what our Operations manager creates for his own reference so he can send to the client and our personnel so they have the info required. The second pic is what I have to record once the timesheets come back from site with regards to working hours to make sure they.. a) Do not exceed 72 hours per week b) Do not work more than 13 days in a row c) No more than 14 hours door to door I can review a, b and c above manually if needed but what I really need is that when the Operations manager creates his sheet, he can access the names of personnel via means of a pull-down menu which automatically pulls in their reference number and telephone number at the same time. Then, he can pick a "leaving home" time (24 hour clock in 15 minute in 15 minute intervals), "arriving home", "leaving site" etc etc. The week ends on a Sunday so will need a calendar pull down menu as well with every week end option as the Sunday. The data on the "data input" needs to look similar to the pic attached as this is Whatsapp'd to staff so they can read it easily.
