Intranet tool for quote/estimation (Classic ASP ONLY)

I'm looking for a freelancer to create a new application/portal using CLASSIC ASP (vbscript) within a timeline of 4/5 days (test included).
Informations are sufficient to provide an estimate (times and costs).
No .Net, no C#, no PHP, no Python, no third party controls, no compiled objects.
Vbscript only. JavaScript client side if really needed.
The use of free objects/sources freely available on the internet is really appreciated (and reduces testing and development time).

I would like to create a web application (only in ASP Classic/Vbscript, no .Net) that manages, after login
the filling out of a web-template that will be used to print a quote/estimation.
NO backend.
The form must have 2 sections:
The first one (see picture in attachment) manage general data as
- my (company) data
- customer data (that will receive the quote)
- quote number (progressive, from 1 to....)
- today date
- expiration date
- options (if any)
The second section (see picture in attachment) manage details as
- list of items (selected from a drop-down list)
- possibility to add and delete items
- additional notes/description
- quantity, Price, VAT
- Total (automatically calculated, using javascript)
- final note (bottom part)
- final recap of costs (with and without VAT)
When the quotation is completed:
- SAVE into db
- prind as PDF (see example of templates, in attachment). Template must be choose/changed by users.
- send the PDF via email
A grid HTML on top will show (with paging) the list of quote saved.
Clicking on each record, the textbox of the form must be filled with data of record selected by user.

Layout design (that must be responsive) is included into this project but you can follow my example in attachment.
Browsers in use are Firefox and Chrome. This application will work under IIS 10 (Windows).

User registration not needed: users will be invited and they receive user/pass to access.
LOG of activities done by each user is needed (a DB table with: who, date, time, what).
A form to show LOGs is needed too.

"MS Access" 97/2000 DB is needed (or you can use what you prefer BUT you have to export in .mdb the database file for our test).
Application will be tested and used into an Intranet.
The application will work into an intranet (no internet)

Please pay attention on application performances.
Pay attention to the budget limit.
No upfront. No payment before successful completion of all tests.
Source-code required for test
Unnecessary images, files, libraries, ... must be removed.
The source code is required to have a good level of comments, to allow bug-fixing, code maintenance and possibly future evolution.
Bonuses are provided at the end of the project for compliance with the timing and quality of the software.

Proposals from programmers who do several activities in parallel will not be accepted: the quality of work could be low.
Start as soon as possible - daily update is requested (showing progress), to avoid time loss and misunderstandings about requirements.
Without daily update with project progress, project will be un-assigned
