Safety survey

Require a high-fidelity prototype with logic regarding a safety questionnaire. Require some basic workflow and business logic for go / no go decisions. Illustrate selection and check list process that is tracked by specific users that is part of the process, and where tasks are handed of to team members that can be tracked by Biometrics(user id) 1. Log in page – User ID information and/or Biometric simulation. 2. Profile page for the user with basic information – Name, ID, Job role, date/timestamp and menu item selection. 3. Declaration page – User declaration with basic checlist items, business logic on radio buttons for go/no go decisions with resolution requirement before the user can proceed. If there is a no go decision, escallation to manager with a call to action 4. Completion of checklists 5. Reporting page to indicate pre-determined KPI’s to showcase faults, sucesses, severity categories, response times - to be elloborated on further
