Food establishment operators in the City of Pasadena are required to obtain permit(s) from the Health Department. These downloadable documents should be completed and returned to:

Sec. 15-22. Permit

(a) It shall be unlawful for any person to construct, operate, or maintain any food establishment as defined by this article within the city unless a permit for that establishment has been issued by the health department.
