About us

At Union Community Care, our purpose is to spark equity through patient-led healthcare that welcomes and strengthens our communities by integrating body, mind, and heart. We envision vibrant and healthy communities supported by inclusive healthcare that embraces each member’s unique culture, needs, and values, and emboldens them to make healthful choices that fuel their well-being and the well-being of others. We believe in whole health. This means we address and heal disease but equally important, we work at the causes of the causes, the social ills that must be addressed to achieve true equity. We listen, learn, and embrace the complex lives and unique strengths of our patients, and we work hard to break down all barriers to care. This means we look through a grassroots lens. We connect with our communities because we are our communities. Each of us is a neighbor, a friend, a family member, and together, we are a trusted community health center. Union Community Care is part of a larger movement of Federally Qualified Health Centers (FQHCs), which are community-based, non-profit organizations that provide a full range of primary and preventive healthcare to their communities. FQHCs are created by individuals in a community coming together with a mission to improve access to healthcare for those who are looking for a health home, those who don’t have health insurance, those who are under-insured, and those who are under-resourced, medically underserved and vulnerable. Every FQHC is different, but they all care for patients in a hyper-local, culturally-welcoming, and free-to-be setting.

    📢 We were there for PA CHW Rally Day! 📢 Community Health Resource Coordinators from Union Community Care joined over 170 public health advocates and allies at the State Capitol earlier this week for the PA Community Health Worker Collaborative’s 2nd Annual Rally Day! It was a day to share stories and highlight the impactful role they have in connecting you to the resources you need to get and stay healthy, including transportation, housing, financial assistance, food access, and healthcare coordination. In addition to hearing from The Pennsylvania Department of Human Services Secretary Valerie Arkoosh, PENNSYLVANIA DEPARTMENT OF HEALTH Executive Deputy Secretary Kristen Rodak, and Pennsylvania Community Health Worker Collaborative Executive Director David Wiles, our team also met with PA State Rep. Ismail Smith-Wade-El and PA State Senator Ryan Aument. The event helped raise awareness of the importance of continued funding for Community Health Workers across our state.

By walking side by side with you through your health journey, they serve as a bridge between you and your care team, making sure you feel supported and understood.     ⭐ Double the Honors: Gold + Silver for Our Care Teams! ⭐ We're sending a loud and proud shoutout to our care teams who were recently recognized by the American Heart Association & American Medical Association for working hand-in-hand with our communities to support heart health and well-being! 🥇 Gold+ Recognition: Our Lancaster Downtown and Lancaster SouthEast teams were honored with the Gold+ award for guiding more than 70% of community members with high blood pressure to achieve control. 🥈 Silver Recognition: Our teams at Lancaster Bright Side, Lancaster Grandview Plaza, Lancaster Reynolds Middle School, Lebanon Medical, and New Holland Welsh Mountain were recognized with the Silver award for supporting healthy, vibrant communities by monitoring blood pressure and providing education on maintaining long-term heart health. Both the Gold+ and Silver recognitions are powerful and practical examples of how we are making healthcare accessible and empowering our communities to take control of their health. The teams prescribed low-cost blood pressure cuffs through our pharmacies, and with a collaborative, team-based approach, they provided community members the personalized support needed to effectively manage their blood pressure. 💙

    🎉 Grant Announcement! 🎉 We have received a $600,000 grant from the Health Resources and Services Administration (HRSAgov), HHS to expand Behavioral Health Care for our communities! What does this mean for our communities? While we’re still finalizing the details of this expansion, we know the grant will help us increase access to Behavioral Health services, with a special focus on supporting individuals facing substance use challenges.     When you "Share Union Care," it's powerful. 💙 When Cariney Figuereo found out she was pregnant, she could have gone anywhere. But her choice wasn’t just about finding a doctor—it was about finding a place that felt warm and welcoming. So, she turned to a trusted friend who guided her to Union. "The care I’ve received here feels personal," Cariney said. "It feels like they really understand me and what I need." Excited about her growing family, Cariney proudly shared her experience with friends, so they too can feel the warmth and support she’s found at Union. "I am looking forward to Union walking with me through my pregnancy," she added. "I feel so supported that I’ve already told another friend who is pregnant to choose Union, too."
