To bring peace and dignity through life’s transitions by providing comfort, care, and support to people in need.
About us
Bringing peace and dignity through life's transitions by providing comfort, care, and support to people in need. Snowline, established in 1979, is a nonprofit, community-based organization dedicated to serving our community by enhancing the lives of those with chronic, serious illness and patients nearing end-of-life. The care provided to patients and families is carried out by Snowline's Interdisciplinary team of professionals, including physicians, nurse practitioners, registered nurses, licensed vocational nurses, medical social workers, spiritual care providers, hospice aides, and volunteers. Snowline's dedicated staff are experts in delivering end-of-life care, with many earning advanced certifications from the Hospice and Palliative Credentialing Center and the Hospice and Palliative Nurses Association. Snowline is a proud Employer Partner with the Hospice and Palliative Nurses Association and encourages our team to seek advancement and certification. Hospice benefits are included in Medicare and Medi-Cal and most health insurance plans. The uninsured and palliative care recipients are also served at Snowline through funds from the proceeds of Snowline's thrift stores, a recycle and repurpose processing center enterprise, individual donations, fundraisers, and the United Way. At Snowline, neither patient nor family will ever receive a bill for services. We serve the western slope of El Dorado, Placer, and Sacramento County, including Sacramento, Folsom, Fair Oaks, Elk Grove, Roseville, Granite Bay, Fair Oaks, Carmichael, Lincoln, Rancho Cordova, and Rancho Murieta.

At Snowline Health, we recognize the importance of mental health, especially when facing the challenges of serious illness, grief, and loss. Caring for emotional well-being is just as crucial as physical health. Snowline provides compassionate support not only to our patients but also to their families, ensuring they never walk this journey alone. Whether through counseling, support groups, or just being there to listen, we strive to offer peace, comfort, and understanding when it’s needed most. Let's break the stigma around mental health and remind everyone: it's okay to not be okay. Asking for help is a sign of strength, not weakness. If you or someone you know needs support, contact us at Snowline. We're here to help. 530.621.7820 or grief@snowlinehealth.org #MentalHealthAwareness #SnowlineHospice #CompassionInCare #EndTheStigma #YouAreNotAlone #SnowlineHealth #griefsupport

Snowline Health had the opportunity and privilege to present at the Aging Well Symposium this past Saturday in Orangevale! Tami Grigoleit (Director of Quality Assurance and RN) presented on the following topic: “Palliative Care vs. Hospice”. Thank you, Tami, for the informative presentation to our community! The purpose of the Aging Well Symposium and Resource Fair is to support families who are caregivers to their aging loved ones. Whether you are caring for a spouse or an aging parent, most families struggle with knowing what they need. #SnowlineHealth #AgingWellSymposium #PalliativeVSHospice #SnowlineSupportingOurCommunity #SnowlinePalliativeCare #SnowlineHospice #SupportingFamilies

What a Great Turnout at the Heritage & Four Seasons Resource Fair! Snowline was thrilled to participate in today's event in El Dorado Hills! It was wonderful connecting with so many community members and sharing information about our programs, including the Dementia Connection, the palliative care program, and our grief support services. A big thank you to everyone who stopped by our booth to learn more about how we serve the community (45 years and counting). We’re proud to be a resource for those in need of hospice and palliative care. -Suzy & Monica #SnowlineCares #CommunitySupport #ElDoradoHills #ResourceFair #HospiceCare #CompanionPetsProgram #HeritageElDoradoHills #FourSeasonsatElDoradoHills #SnowlineHealth #DementiaConnection #GriefSupportServices #PalliativeCare
