It's important for each discipline to have at least a basic understanding of what the other disciplines provide. And the value of their contributions. This can help improve clarity, team continuity, and client outcomes.

Definitely. Having a defined role makes you understand that each member of the team has something to contribute to the provision of care for the patient. Having a defined role, making sure not to overstep the boundaries of your practice, and keeping mutual respect for each team member encourages effective collaboration between the interdisciplinary team members. 

Clear role definition is crucial, but recognizing and affirming the specific contributions of each discipline can serve as a wonderful lubricant for perturbed social dynamics. For example, if you see a colleague, like an MD, nurse etc..., making a good move, take a moment to express your appreciation. A simple comment like, "I've seen what you did with that patient and I really liked it," not only boosts morale but also fosters a supportive, wonderful & cohesive team environment.

To have a representation of what effective teamwork looks like, there has to be open communication. Open communication or dialogue helps carry everyone along, including the patient and the carer or family members. Keeping an open dialogue does a wonderful job of reducing conflict of interest, encouraging teammates to bond and trust each other, and mitigating the risk of harm to team members or to the patient.

I totally agree with this. Some of the important factors that make up a goal are: 1. There have to be short- and long-term smart goals. Secondly, the team involved has to be involved in or well aware of the goal; this is where open communication plays a role too. Then, thirdly, the patient has to contribute to forming such a goal. 

No, if a professional is goal setting in isolation of the patient, the carer and the family, the plan becomes nothing more but what the organisation wants or can offer. Firstly collaborate with the person and look at the resources they have. Identify with them what support or assistance they feel would be helpful they they can’t access. Explore together what help is available. If they ask for advice talk about what would make a difference for them, then once you have their goals, you can start to explore how as a team you can help meet those goals.

Completely agree that regularly checking a patient’s progress helps adjust their care plan. Using clear measures and feedback from both the patient and care team is so important. In my experience with our healthcare customers, machine learning and AI can also analyze data to spot issues early, personalize treatment plans, and improve care by adapting to the patient’s changing needs. It also reduces caregiver burden and increases team confidence in decision-making.

For example, it might be necessary to explain the differences between the services provided by a physical therapist and an occupational therapist. As the care plan is developed, ensuring that patients know who to contact for specific needs will help achieve goals more efficiently.

It's important to remember that professional disagreements about how to approach a client issue need to be treated differently from interpersonal conflicts between team members. Both types of conflicts arise on teams. And both need to be addressed. However, interpersonal conflicts cannot dictate client care.

Having regularly scheduled, semi-structured meetings to discuss the goals, progress, and determine next steps creates the space for open communication and promotes interactive collaboration. The group can brainstorm, build off of each other's ideas, develop rapport to achieve the most effective outcomes.
