Always remember that there are many things we do because they have been always done a certain way. Map the process regularly and ask why are we doing each one of the steps. Does it add any value to the patient? If not, ask yourself: should I do it because of a legal/administrative requirement? If the answer to both questions is a no, eliminate it. Repeat. And do it overtime. You’ll be amazed by how many steps can be eliminated over time and facilitate patient flow in the hospital.

When I started implementing continuous improvement 20 years ago, I made a massive mistake I hope you can avoid: The first step in each project was to ask the process owner (responsible staff member) to walk me through each task from start to finish so that I (the "expert") could create the plan to resolve the constraints and implement it (Lean/SixSigma 101). Why did the plan always come way short when I implemented it?!? Because during the process of ideating the solution, I did not invite the process owners to participate :) If the process owners do not believe it was their idea too, they won't have ownership, and after you turn around, they will go back to their old way or sabotage the new one with great excuses.
