Failure can be a stepping stone to success. The unfortunate truth is that many projects fail; the question becomes, "what did we learn from it?" If we wanted to "fix the blame," we are in a low trust culture. So how do we approach failure? How do we learn from our mistakes in a positive way? First, non-attribution! What failed, or what failed first? Did we understand the requirements? Had we taken the time to make each requirement explicit? Was it singular; did we turn it into a simple and explicit statement everyone could understand? Was the requirement measurable, did we understand the success criteria? Did we understand dependencies, sequences, and precedence? If we don't know the requirements, we shouldn't expect success!

Does our team or project culture have a supportive feedback mechanism? In dialogue people are encouraged to speak and to listen. This is not a blame game, listen without interrupting, without verbal or non-verbal attacks. Use a facilitator to lead the session.
