Some fixes to extract newer versions of Fallout 76.

Updated readme and convert_fuz_to_xwm.ps1

- Rewrote convert_fuz_to_xwm.ps1 so it can natively handle fuz and xwm file formats (no 3rd party tools needed anymore!)
