When I first tested, I put the form into a Cover blocks, but it appeared broken. The success message didn’t appear because the text is black and my broken cover block was also black instead of an image.

After trying the cover block again the next day, it worked as expected. As a side note: I did have to choose a different Page Template in the Bright Mode theme to get the expected Full Width version of the cover block.
