import page (xls to mysql) in wordpress site

Build an import page where i can upload an xls file and import the data in the mysql database. An example of the xls file is attached. On the website I want different choices to show the data: show domains and price in alphabetical order of "domain" column and attach to page [login to view URL] show domains choosing a category and attach to page [login to view URL] every list line starts with a checkbox, needed to order this domain. Below the list the visitor must fill in these fields: Naam Adres Postcode Woonplaats telefoon e-mail The complete form must be submitted to an email adress (order@[login to view URL]) including the domains which have an active checkbox.
