Save the table as CompanyFeedback.
Step 3: Link the Tables
Go to Database Tools > Relationships.

Manager Review Form
Use Reviews as the base table.

Lock EmployeeSelfFeedback to prevent editing.
Step 5: Add Sample Data
Enter sample employees, criteria, and reviews to test the system.
