Comprehensive Medication Tracking Google Sheets Script

I want an app script written for google sheets that will allow people to enter these fields:
Client name (required field)
Client ID # (this one not required field)
Medication Name (required field), dosage (required), time and amount to take (required: such as example: Morning 1 tab daily), prescribed date (required), refill date (required), doctors name (not required).
There needs to be the ability to put in more than one medication (hopefully without reentering the clients name each time. There would be buttons for Searching, that would bring up a client's entire medication list including discontinued meds. A modify button, so that when a new refill comes in for an existing client one can simply modify the refill date of that med. and a button or option to check to discontinue a med (which would group it under discontinued meds for the individual client. I would like to be able to print a list of all clients and their medications grouped according to time of day. (morning, lunch, dinner, night). This would be a spreadsheet that can be viewed by all. I also want to be able to categorize clients as active or de-active after they leave. Yet, one could still search records be clicking active or de-active client.
