The current problem is that when people manually inject (using liquid from a glass ampoule) into an insulin syringe, the needle part is not long enough to draw the liquid from the top up. When you turn the glass ampoule upside down, the gravity and air stop the liquid coming out and you can draw the liquid into the syringe from the bottom and the liquid just drops down as it fills the syringe. Right now, we achieve this by using sticky tap on a plastic container.
