Very Simple

I need a very simple PHP form script. I have 4 fields that I need information: 1. NAME* 2. PHONE 3. EMAIL* 4. Message box * = required I currently have created the non-working form in HTML. I need it to be working. All fields should be sent to me via EMAIL in the following format: CONTACT INFORMATION: Name: ##name## Phone: ##phone number## E-Mail: ##email address## Message: ##Message## ** I NEED STEP-BY-STEP DIRECTIONS ON HOW TO IMPLEMENT THIS. I ALSO REQUIRE STEP-BY-STEP DIRECTIONS ON HOW I CAN IMPLEMENT THIS SOLUTION IN THE FUTURE TO OTHER PAGES ** Thanks. I NEED THIS COMPLETED NOW, AS SOON AS POSSIBLE.
