Make work a questionary (dynamic positioning of questions, receive answers etc)

Hello, I have a questionary in [login to view URL] It isn't working because I did a simple questionary in HTML and absolutly no programming. What I want: 1) Proccess this survey, reports with filters by region (question 4), male/female (question 2) and age interval (question 1, like 20 - 30). 2) Dynamic positions of answers in those questions: 5, 8, 9, 11, 12, 13 -> Answers must be showed in aleatory positions inside those questions. (please be sure you understand this, my english is not the best one) 3) Multiple choose in question 12; 4) Give report of email lists with those who want to receive survey results (no automatic sending, just list), who didn't and all emails. PS> Who want to receive results is the last checkbox after question 13. I only had this [login to view URL] file, freelancer will did all this, mySQL files, php files, insert form and sucess page.
