Customizable Online Booking Form Development

I need to develop an online form that collects information related to clients booking our services, and provides a summarised version in a printable form / table for easy reference. I have a template of how I want the summarised form to look like and a list of all of the questions and options available for the client to choose from. * The form initially collects data about a single product, but needs the ability to add additional products to the order once all the details about the first item have been collected * The form needs to be able to have selections that affect what information is gathered - ie of you choose option 'A', option fields '1,2,3' are generated. If you choose option 'B', option fields '4,5,6' are generated. *One entry will require the calculation of volume (in litres) to Number of bottles (depending on the size selected) and number of dozens (rounding to the nearest multiple of 12) * There needs to be a number of drop down lists of selections for clients to choose from, and these lists need to be easily editable by me in the future after the form is published online. * The summarised table that is generated after the client has completed filling out the form needs to be automatically emailed to myself and the client. The client also needs to be able to go back and edit and resubmit the updated online form if necessary. The form needs to be cross platform, adaptable and useable on all devices. In the future I would also like to have the option to have this information put into a extended version of the clients booking form with additional fields that we can fill out during production. This is not needed right now, but having this flexibility for future development is important. The form needs to be placed on our existing website.
