Integrating old php booking form into new template

I have a simple 1 page booking form [login to view URL] and need to put this into my new template page [login to view URL] The date fields should be replaced with jQuery datepicker and display as dd MM yy The number of booked days is calculated after the POST and used to email the guest. My main problem is lack of experience with jQuery /Javascript and for an experienced programmer this should only take about 1 hour. As I wrote the original booking form myself, I can easily provide all the PHP files. The final layout does not need to be perfect but the date functions should work. If you know how to display datepicker in French and German, this would be an added bonus. For now I simply need an English version.
