Add Annotations to PDFjs and Fill Form

This project is to add Annotations to a PDFjs project for inhouse use. PDFjs can open PDF forms. Here is what I need. Our project is build on laravel but that should not matter. All the work I need done is in javascript.
1. We need to open the PDF Form via a link. -> PDFjs allows this.
2. We have a few fields in the pdf that we need to fill them out during the load. I guess PDFjs has a class AnnotationStorage that we do not know how to use. There are 5 fields that we need to fill out before we load the form.

3. We need two add the ability to the top menu of PDFjs to import an image and signature panel.
I found this code that seems to do the image but not signature.

4. The last part, PDF js allows to download the file but we need to send it via post back to the server.
