Some alterations, and additions, required to newly created PHP form created by Kun01.
Job for Kun01 if bid satisfactory.
I should add here that there are 13 items that require attention on this job - a few of them will involve a bit of coding ie Adding 3 additional fields of subtotal, gst and discounts to a floating 'price total' box. Another big one is having three options available (through radio buttons) that will result in different text being displayed to the viewer.
So please take it as being a few hours of work involved with this job... I would think.
