GNet Dynamic quote form

Stand alone three page dynamic form - for quotation purpose. Needs to be responsive. Approx 15 static form fields and 5 dynamic (database values) form fields to show for user to select. Cart style quote box in right column to show database records related to user selection and totals. Cart to update after each user selection.
