MT4 Custom Indicator: High & Low, Price & Time

We need a very simple Meta Trader 4 indicator which will show us the highest and lowest prices of each bar, and more importantly the time of each high and low occurrences. There won't be enough space to do it on all bars, so it would be nice if you can make it relatively visible, and set up an input parameter which gives us the option to apply the indicator only to certain bars, or cyclical to every particular time e.g. in every 12:00 bar. The indicator must also be applicable to live price bar, creating new highs and lows as they happen. Please see attached for the outcome of the indicator that we're looking for. The numbers are hypothetical. Mathematical formula may not be needed. This is simply to display the price and the time when the high and the low of a bar occur. We try to be as specific as possible, but if you need further details, please ask us in advance to avoid any confusion.
