Cutting pieces for custom cabinetry and shipping them to me.

Because a competitive price is important, maybe the bidder might want to do this during his idle down time. That is, there is no hurry, and so even though the project may only take 10-30 hours to complete, it is okay if it takes 30-60 days to complete so the bidder can do it during his down time. But it does need to be completed within 60 days of start date.
There are 4 cabinets total, 2 of which are mirror opposites of each other. The cabinets will be for a loudspeaker system. There will be two cabinets for woofers (low frequencies) and two for the midrange speakers and tweeters (high frequencies). The cabinets for the midrange-highs will sit on top of the woofer cabinets. Each cabinet will be, after I assemble them, about 38" wide x 38' deep x 30" high. When the one cabinet is sitting on top of the other they will be a total of about 60" high. Make sense?
What I need cut are sheets of 1/4" plywood, 1-2" thick XPS hard polystyrene foam board, and 1/4" walnut veneer cut to size. Because the cuts are curved, they will need either a CNC or laser cutter than can cut both 1/4" thick plywood/veneer and 1" or 2" thick XPS polystyrene foam board. The patterns are in Adobe Illustrator, the standard software used on CNC and laser-cutting machines. So I can know of your capabilities, please send me photos of your machinery, brand and model numbers so I can confirm your equipment is adequate. I'll supply the materials from Home Depot.
The sheets of each will come in 4' x 8' sheets, for example, 4'x8' sheets of plywood, XPS Foam, etc.
I need to get a fixed bid that will itemize both cutting and shipping costs to me in Utah, Zip 84770, USA. I would like to know who has the equipment who can do this work, general capabilities and experience, and a fixed bid for the cost to cut, via laser, sheets of 4'x8' sheet of basic 1/4" thick plywood, 1" or 2" XPS polystyrene foam board, and 1/4" veneer.
Assume that there will be:
a. 10 sheets of 1/4" plywood;
b. Preferably 5 sheets of 2" foam board, or if you cannot cut 2" thick sheets, then 10 sheets of 1" foam board; and
c. 2 sheets of 1/4" walnut veneer. The materials will come from Home Depot as purchased by me, deliverable to you.
It will be your job to securely package them and ship them to me in Utah, zip 84770:
a. The 2 largest pieces - the grilles - will be fragile and will need to be packaged inside two sheets of foam board, then inside a rigid crate or unbendable box: 1/4" veneer cut into a detailed filigree grille with each piece being 24" w. x 62" long, and estimated weight is about 4 lbs. total + shipping container.
b. The next largest will be 16 pieces about 38" x 38", and estimated weight is about 20 lbs. + shipping containers.
c. The largest of the remaining pieces will be an average of about 15" w. x 30", and estimated weight is about 70 lbs. + shipping containers. The number of containers or boxes may depend on the number of pieces put into each box.
d. The shipment should be insured against damage in the amount of replacement costs.
e. Please give me the fixed bid assuming the above sizes and weights, including the size and weight of your estimated packages for shipment. I would like to get this itemized in the bid, broken out as a separate item, so I have a sense of how much it will be, and we will make an adjustment for over/under then adjust for actual costs at the time of shipment.
f. Please specify how you would propose shipping them (wood crate, cardboard boxes, etc.).
There will be four Progress Payments based on photos of the following: first payment (25%%) when all the non-grille cutting is done; second payment (15%) when there's a photo of the 24" x 60" grille is finished being cut; third payment (20%) when everything is packaged and ready for shipment; and the fourth and final payment (40%) after I receive the shipment and everything is satisfactory. They will be based on photographs of the cut pieces, numbered and stacked per the Adobe drawings.
I would like an itemized breakdown of all costs of the bid so we can follow the above program.

The shipment should be securely packaged in wood crates, though perhaps some pieces may be adequately shipped in high grade cardboard boxes.
A prototype will not need to be approved before proceeding with the full batch, but I will need to see close-up photos of the items.
Either FedEx or UPS for shipping.
Standard precision laser cuts are acceptable for the required pieces.
