GIS Based On-Boarding Form

We are in need of a GIS-based Quote Request form. We currently use a combination of JotForm, Sharepoint and Zapier in order to integrate various processes. I would like to create a prototype web-form that would ask the following questions: Form Page 1: Is your project located in Florida (Yes or No) -- IF "Yes" Form field opens: "Please select the County where you need the survey done" (Dropdown of Florida Counties) - A pre-query will occur for that County, selecting only the parcels for that County. "Please enter the address or parcel number you need surveyed." The search bar will pre-fill after the first 3-4 characters are typed in. (Next Button) Page 2: - Map showing the parcel selected "Please confirm this is the parcel you need surveyed" (Back) button or (Confirm) button Page 3: - Please provide us with your contact information First Name Last Name Phone number Email Address (Submit) Once submitted, the GIS will take a "screengrab" of the selected parcel, with the parcel number showing somewhere, as part of the graphic. Perhaps 7 inches Wide and 7 inches Tall The resulting form data and graphic will then be passed along to Sharepoint via Zapier or can be placed in a database, and then also passed along to our Sharepoint List For testing purposes we can use a subset of some parcels from the following shapefile.
