Hello, I need a coder to make a form from a printed design (attached), it needs to be virtually the same format as the image attached, but I would like the form to be to be filled in and then printed out on an A4 sheet of paper - the yes / no choices should be a drop down of which you choose an answer, these must clearly be shown on the print out. The text for the questions should be editable, and the space for adding text should be enough for all circumstances, in other words the form questions should drop down if space is needed, to another page if needed. The form itself should unique, with a reference number that is generated automatically on creation of the form, it should be able to be saved externally from the program, and it should be able to save to pdf, ready to be emailed or printed.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
## Platform
It can be any programming language (as long as it works with XP and Vista), or it can be with any office 2007 application such as InfoPath or Access. I am open to suggestions.
