Automate email campaign with opt in buttons

We are looking to develop an email campaign that we can send to clients with embedded buttons. Essentially we need them to accept our terms that they have previously accepted. I imagine there will be one button on the email and if possible, once they click it a pop up will appear with 2 or 3 other questions (this could simply direct them to a website that will hold these questions) We would like their details to go from page 1 to the ongoing pages to help instill confidence and user experience. so if the email says 'Hi John' then when he clicks, the next page will say 'John, please tell us what's important to you' we also need the ability to obtain the below 1. Tell us who has accepted the terms 2. Tell us who hasn't 3. the ability to email the ones who haven't as yet as a reminder to do so please contact me with suggestions on what we can use.
