289733 Custom CB Affiliate Script

I need a custom ClickBank script made to where my affiliates can join, so basically, I would need a form for them to signup and store their Name, and affiliate ID in a database. Then in the affiliates area there would be check boxes for which product they wanted to promote such as: 1) prouct 1 - But it will actually be th ename of the product. 2) product 2 - But it will actually be th ename of the product. And so on.. They then can click which product they want to promote and hit submit, it will then take them to the affiliate tools section with that products tools and the affiliate url.
