Customized Form in PHP with Secure SQL Database including online deployment
We are seeking to develop a aesthetically pleasing, and secure web-based form integrated with a SQL database. This form will facilitate and receive applications for a monthly token draw for 10,000 lucky users, each winning 1,000 tokens(Max application per month 1 Million). Selection of winners will be taken care separately and is not part of the project.
The project requirements are as follows:
User Authentication and Entry
Implement Telegram API integration for user login, ensuring unique entries.
Display the current draw month prominently after login, e.g. Heading, " Draw for the month of January 2025."
Prevent multiple entries:
Check if the user has already submitted an entry for the current month.
If so, display: "User has already submitted an entry for the current month. Kindly return in the next month to enter the next draw."
Form Functionality
Automatic monthly transition:
Collect data on a monthly basis.
Shift to the next month at UTC+00:00 on the first day of each month.
HarmonyOne address collection:
Validate the address format (must begin with "one1" and be 42 characters long).
Remove leading and trailing spaces.
Disallow spaces within the address.
Check for duplicate addresses within the current month.
If a duplicate is found, display: "Address already entered for the current month. Multiple entries with the same address in a month not permitted."
Private key and seed confirmation:
Ask question "Confirm whether you have securely saved the privatekey and seed for the above Harmony address?"
Provide two radio buttons:
a) "Confirmed, I have securely stored the private key and seed and it is accessible at will."
b) "Declined, I have not stored the private key and seed."
If "Declined" is selected, display: "Loss of private key and seed will result in permanent loss of tokens with no recourse. Only use an address where the private key and seed are securely saved/stored."
Make all fields mandatory.
Referral System
Implement a dynamic URL structure: draw_entry.html/ref=0
Allow users to replace '0' with their HarmonyOne address for referrals.
Validate the referral address format before form loading.
Display error message for invalid addresses: "The affiliate address is not a HarmonyOne Address. Retry using a HarmonyOne Address."
Allow unlimited use of HarmonyOne addresses as affiliate addresses.
Include a "Reference address" section showing the HarmonyOne address from the URL.(after the heading on top of the page).
Add a "Refer and win" dropdown with the following information:
>Earn one token for every referral (minimum 50 entries per month required).
>Win 100 tokens for each referred friend who wins in the draw.
>Obtain monthly rewards based on Twitter rewards poll results.
Add second dropdown Menu - How to obtain a referal link?
> draw_entry.html/ref=0 replace the 0 in the address with your HarmonyOne address.
eg. if your HarmonyOne address is 0xA110BaaD21508BB8edf19f48b5F211d9d05Bc7ba your referal link will be draw_entry.html/ref=0xA110BaaD21508BB8edf19f48b5F211d9d05Bc7ba

Include the following message at the bottow before submit.
Important Note
"It is recommended that you acquire at least one dollar of HarmonyOne tokens on the mainnet for gas fees to move tokens upon receipt. Alternatively, you may obtain complementary gas for up to 100 transactions (under normal load conditions) along with 100 tokens from the purchase of digital products from our webstore."
