HTML PHP form to multiple text files

I want an HTML page with 4 text boxes and 4 submit buttons. I want each text box to save to a different text file (text1, text 2 text3 and text 4 when the relevant box1, box2, box3, box4 button is pressed and a success or fail echo shown for each one individually. I also want a browse and upload button where I can select all images from a folder NOT one by one to go to a specified folder on the local PC
