If your registration card has been lost, stolen, or mutilated, you may get a replacement registration card for a fee.
To obtain a replacement registration card:

- Complete an application at a DMV Kiosk. Bring your license plate number and last 5 digits of the vehicle identification number (VIN).
OR
- Complete an Application for Replacement Plates, Stickers, Documents (REG 156) form.
- Complete a change of address online if your address has changed.
- Pay the required replacement fee.
