KIOSK for patient registration

I would like to construct a registration program for patients to enroll in my practice on their cellphones or PC's, and/or on an iPad when present in the office in person This program will be for NEW patients (NP) and for RETURNING Patients (EP) There are two separate programs that need to be populated with information. I do NOT Have API's for these programs. 1. A financial program that stores demographic and financial information (PM) 2. A medical records program (EMR) that stores medical information The program that you construct will extract information from the responses to the questionnaire and populate the corresponding fields in these online programs with that data. NEW PATIENTS New Patients (NP) will receive a link or a page that allows them to enter relevant demographic information. (please see representative fields in the attachments) Each question has a corresponding field in PM. These data will need to be 'typed' into the PM automatically by the program. Answers to medical questions (see attachments) will be uploaded into the corresponding fields in EMR in like manner. When a new patient (NP) is created in PM and the fields are populated with the data from the questionnaire, there is a sequence of steps that the program will perform in EMR that finds the NP and transfers the data from the PM to create the same entity in EMR. Then the program will populate the data from the questionnaire into the respective fields in EMR. ESTABLISHED PATIENTS (EP) Rather than a blank form, the returning patient (EP) whose data already exists in PM and EMR will receive a questionnaire with the existing data already populated in the form . The patient can then review and update this information. Each field will be shown and allow the patient to check "no changes" or to make changes in the repsective field. - If changes are made, the revised data will be uploaded into PM and EMR as for a NP - If no changes are made, the patient can verify that no changes are needed. A note will be made in a field of the EMR that records the date and that "patient reviewed all data and made no changes". * There will be links for the patient (NP or EP) to upload their insurance cards (both sides) and their driver's license or photo ID) The upload should be made possible from a cellphone picture or scanned image. * There will be forms that must be signed (?electronically?) by the patient acknowledging certain terms or conditions. Some of the forms will be relevant to all patients. Others will be presented based on information that is provided in the questionnaire (e.g., patients with Medicare will receive the Medicare document) (I have attached examples of these fields, but they are only for demonstration, they will need to be revised as the program develops). The program should allow me to add / delete / or move fields in the questionnaire at any time in the future. ALL OF THIS INFORMATION NEEDS TO BE: - stored in a database apart from being populated into the two programs as backup. This must be a serachable datatbase (e.g., Excel) that will allow review and transfer of data at any time. - sent to the practice email (also in a form like Excel or the like) in case a field is not properly populated or the data does not integrate properly - sent to the patient (as a document) as confirmation of their enrollment The program must be robust and durable. The programmer can select the technology used for this purpose. It ca be hosted on a website or other platform. Time to completion and cost are relevant. Please see examples (NOT all inclusive) of some of the fields and a rough schematic of the process. These are merely to represent the content.
