FY 2024 School District Annual Financial Report Package

The FY 2024 AFR Forms zip file contains forms and instructions for completing the school district AFR; 2 school-level reporting (SLR) forms based on district size, Food service AFR; Classroom Site Fund (CSF) Narrative; and Desegregation AFR for districts that budget for desegregation monies.

The FY 2024 AFR package includes the Work Sheet for Determining the Appearance of Supplanting with CTED Monies. Member districts are required to submit the completed form to the CTED’s governing board and the Arizona Department of Education.

Select the zip file and extract the files to C:\SDFORMS\. Choose the appropriate school-level file based on the number of schools in your district. In addition to the extracted files, the C:\SDFORMS\ folder should also include the most recently revised versions of the district's FY 2024 budget and FY 2023 AFR files.
