Requirements
Accomplished Condonation Request Form and/or letter request
Authorization from the borrower if application will be through representative
Photocopy of one valid government issued ID of the borrower
Mode of Payment
Cash
Installment up to six (6) months with zero interest
Loan renewal
Notes:
Borrowers may proceed directly to any of the Branch Offices to submit the abovementioned requirements.
A borrower can only avail of amnesty or condonation/waiver of interest charges for his/her
non-performing loan account only once in the lifetime of his membershio with the
Association
