Lymphocyte-depleting induction and steroid minimization after kidney transplantation: A review

Abstract: Steroid minimization after kidney transplantation has become more widely practiced as transplant clinicians seek the potential benefits such as reduced cardiovascular risk factors, improved growth in pediatric patients, and improved compliance with the immunosuppression regimen. Steroid avoidance (i.e. no steroids after the first week) is generally favored compared to later withdrawal. Induction therapy is routine in this setting, frequently rabbit antithymocyte globulin (rATG, Thymoglobulin®) or off-license u… Show more

“…One drawback is that there 2 changes in the immunosuppressive regimens so that the conclusion about steroids avoidance is limited by the need for a "stronger" induction in low-immunological risk patients. [6][7] Similar conclusions were reached when alemtuzumab induction was compared with basiliximab in the 3C study 7 .…”

“…Patient and graft survival was not affected in patients inducted on rATG and maintained on TAC and MMF [44,52]. In a study comprising 124 kidney transplant recipients, of whom 91% were on rATG induction Visger et al, also had the same results as shown by Kuklan [44,53]. Generally in patients…”

“…Information high immunological risk studies, the AR rates were moderate at 13-14% with a remarkably improved graft and patient survival being recorded. However, there is still need for further verifi cation on the consistence of this type of management protocol [44][45][46][47].…”

“…( 5 ) The ATG is a polyclonal antilymphocyte agent, consisting of a wide variety of T-cell epitope-specific antibodies, which induce rapid and profound CD3 + T lymphocyte depletion in peripheral blood. ( 6 ) In the past, wide use of doses adjusted exclusively for body weight and related adverse effects have often led to over-immunosuppression. Monitoring strategies were then developed to minimize adverse effects and reduce costs while maintaining treatment efficacy.…”
