A Delphi Exercise to Identify Characteristic Features of Gout — Opinions from Patients and Physicians, the First Stage in Developing New Classification Criteria

“…Previous studies have also shown people with gout demonstrate apropulsive gait patterns with reduced walking speed and cadence [10,12]. Despite the importance of lower limb and foot muscle strength requirements in major daily life activities, including walking, the strength of foot and ankle muscles in gout is poorly understood.…”

“…People with gout report high levels of foot pain and disability [6][7][8] and experience difficulty in carrying out recreational and daily living activities requiring normal lower limb function [6,9]. Furthermore, people with gout rate walking difficulty as a discriminatory feature of the disease [10]. This is supported by previous laboratorybased lower limb research in which people with gout walk slower with shorter step and stride lengths compared to people without gout whilst walking at a comfortable self-selected speed in their own footwear, even in the absence of current acute arthritis [8].…”

“…Beyond its clinical (and statistical) relevance, the main methodologic concerns rely on the face validity of the items to be included, the assumption of a compensation among items, and the relative response to treatment of single items within the composite score 7. Previous studies explored the relative relevance of items belonging to the OMERACT domains to be included in a composite outcome measure, showing substantial disagreement among clinicians, researchers, and patients 5, 8, 9. Nevertheless, more recently, a preliminary definition of remission has been proposed, suggesting the potential validity of measuring a state within the continuum of disease activity in gout 10.…”
