The North Dakota Department of Human Services reminds people that help is available to assist persons with disabilities to transition from an institution or facility to community living with the support services they need. The agency is hosting a webinar on March 24, 12-12:30 p.m. CT, that people can join by phone or online to learn about community transition services.
The department’s Money Follows the Person (MFP) program has been working with partner agencies, especially the centers for independent living in North Dakota, to help 638 eligible Medicaid members, transition to an integrated community living setting. Of those individuals, 420 persons with disabilities transitioned from nursing homes or hospitals to apartments, family members’ homes or other types of community living arrangements with supportive services. Participation in transitions is voluntary.
This webinar will provide an overview of transition services offered through the MFP grant and other resources. Presenters will explain the services, general eligibility requirements, how services are funded, and how North Dakotans can access community transition services.

The Adult and Aging Services Division works with partners to help older adults and adults with physical disabilities remain living in their homes and communities.
