Hypoxemic acute respiratory failure (ARF) is a condition where the respiratory system fails to provide adequate oxygenation, leading to severe hypoxemia without hypercapnia. It is typically diagnosed in the absence of underlying lung disease or acute cardiogenic pulmonary edema, with pneumonia being the primary cause. The French Intensive Care Society (SRLF) and the French Emergency Medicine Society (SFMU) held a consensus conference in December 2023 to establish evidence-based guidelines for oxygen therapy in ARF. The conference excluded cases of acute cardiogenic pulmonary edema and hypercapnic exacerbation of chronic obstructive diseases. The guidelines, intended for healthcare professionals in various settings, were based on 22 statements addressing 11 key questions.

Toshiaki Iba et al. review the introduction of SIC criteria and its scoring system for a better management of sepsis-associated DIC. Sepsis, a life-threatening condition caused by a dysregulated response to infection, leads to organ dysfunction. Early detection of coagulation disorders is crucial for assessing sepsis severity and prognosis. Despite advances in understanding DIC in sepsis, management progress has stalled, and sepsis mortality remains high, especially when complicated by DIC. The SIC criteria, introduced in 2017 and adopted in 2019, aim to detect early-stage DIC and identify high-risk patients. The SIC scoring system is easy to use in clinical settings and aims to improve the management of sepsis-associated DIC.

In this review, Marlies Ostermann et al. explore the roles and challenges of new biomarkers for acute kidney injury (AKI) in clinical practice and research. AKI, characterized by a rapid decline in kidney function, leads to significant health complications and increased healthcare costs. Traditional biomarkers like serum creatinine and urine output have important limitations. The discovery of new functional and damage/stress biomarkers have improved the understanding of AKI's causes, mechanisms, and severity, enabling earlier diagnosis, better prognosis, and identification of AKI sub-phenotypes. Integrating these biomarkers into routine clinical practice would be essential for personalized AKI management.
