I did a detailed review of up to chapter 3, did many semi-automatic fixes of the rest, and incorporated all the ORA errata. I also figured out why my changes to preface.xml weren't being reflected in the pdf I generated. This is because preface.xml isn't used to construct the pdf. Instead, ch00.xml is used.
