SCDoc: add a plaque to extension help files #2449

How's this look? As suggested at #2446 the word "Extension" should probably be replaced with the name of the extension.

This commit adds a little plaque to the title of every help file coming from an extension, so it is easy to distiguish core from non-core help files at a glance.

will supercollider need a network connection to those URLs?

No. They are unique identifiers for XML namespaces.
