I just wanted to add... ive had these issues, and its been solved, in my experience, by making sure there is an additional file open in vs.
try making sure you have anything else open in the editor; does not matter the file type, and for some reason that seems to prevent the extension from closing every time. Not going to lie, I fought with this for way too long, until I realized that if I had literally any other file open in the editor, the problem was non existent

When adding a new feature (as shown in the figure below), upon clicking ‘yes’, Pythagora exits without any prompt and stops working automatically
