Community Health Service Inc. has locations throughout Minnesota and North Dakota to serve our patients. Once registered as a patient, you can go to the clinic that is most convenient for you, and all of your registration will be accessible at each site.

CHSI also has two mobile unit clinics that travel to surrounding rural communities to 
provide care.
