We look forward to you becoming the next Social Work Care Manager, Part-Time position, 8hr shifts 3 days per week, likely a Tues, Wed, Thursday type schedule. at our 180 bed full service acute care facility. The Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support when indicated. The social worker functions as a liaison between the patient and community resources and post-acute care providers. The social worker assists and facilitates timely implementation of discharge plan tasks and facilitates post-acute interventions identified in the plan of care. The social worker supports patient movement across the continuum of care, identifies/addresses, and tracks barriers to patient throughput.

Responsibilities

Interviews clients and their families and coordinates and plans programs and activities to meet their social and emotional needs. Provides crisis intervention and assists families in understanding the implications and complexities of the medical situation and its impact on lifestyle. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Works under general supervision. A certain degree of creativity and latitude is required. Typically reports to a senior case manager. Familiar with standard concepts, practices, and procedures within a particular field.

Qualifications

Must have 1+ years of Acute Care Social Work experience to qualify

Bachelors Of Social Work Required

Must have AL Social Worker License

Masters of Social Work (preferred)
