Medical Case Manager Location:

Augusta , ME Duties: Shall track medical status of all assigned Soldiers. Schedule profile exams/evaluations and audiology exams at periodic health exam facility. Coordinate Soldier treatment plans with appropriate medical facilities. Correspond with soldiers to obtain information for the Soldier’s case management file, referred by a provider are followed. Manage Soldier’s care from the identification of the condition to when the Soldier is either considered medically ready and returned to duty or administratively separated from the military. Educate Soldiers on resources available to improve medical and dental readiness. Review and track health assessment lab results and contact SM and educate on lifestyle modifications. Update MODS as required on the following: immunizations, designation of pregnancy, eye exams, and medical warning tags. Track medical appointments by communication with unit to ensure Soldier attendance and submission of follow up documentation. Shall prepare medical documentation for Line of Duty (LOD) investigation for units for which they are responsible in conjunction with Health Service Support (HSS). Shall mail all medical records to ensure timely and accurate processing of medical records to and from mobilization sites, 30 calendar days prior to soldiers’ report date. Shall enter individual vouchers for SM IMR requirements into the Automated Voucher System (AVS) when a soldier misses a MRE or SRP.

Qualifications

Shall have at least 5 years of case management experience Minimum of 3 years clinical experience. Education/Certification: Registered Nurse III (RN) License Preferred Certified Case Manager (CCM) Certification. Powered by JazzHR
