The Center for Elders’ Independence (CEI) provides a Program of All-Inclusive Care for the Elderly (P.A.C.E.) - a comprehensive healthcare model specifically designed for low-income, frail seniors. It combines medical and social services along with vigilant care planning and coordination in order to keep frail seniors safely living at home and in their communities. This unique and highly effective model utilizes an interdisciplinary team approach to care planning to drive high-quality, affordable, integrated healthcare services to the elderly.


Position Overview:

Responsible for providing senior leadership of key clinical care and direct participant service programs. Clinical care includes nursing, rehabilitation therapy, and behavioral health in the PACE center and home care settings. Direct participant service programs include nutrition, activities, social work, and participant medical specialist referrals. The Sr. Director will initiate, monitor, and lead nursing and clinical practices and projects within the interdisciplinary team approach to ensure the implementation of quality, cost-effective, standardized, and efficient processes that support the coordination of participant-centered care goals at all PACE sites, and within the appropriate compliance framework.



General Duties and Responsibilities:

  • Provides senior leadership for the functional managers, charged with identifying, integrating, and monitoring best practices in nursing, activities, rehab therapy, nutrition, and social work.
  • In collaboration with operations leaders ensures acceptable standards of care, based on applicable regulatory standards, including but not limited to: California State Board of Nursing, CMS PACE (Program for All-inclusive Care for the Elderly), DHCS ADHC and Medi-Cal, Commission on Dietetic Registration, Rehabilitation, Behavioral Health and Social Work Standards for practice, as well as CEI’s standards, policies and procedures.
  • Collaborates with the Chief Medical Officer, Sr. Director of PACE Operations, and Director of Quality and Process Improvement to improve operational efficiency and clinical quality outcomes of the PACE medical clinics..
  • Under the direction of the SVP, Operations collaborates with other CEI leaders to drive consistency of operations and performance improvement. Partners with colleagues to identify and implement innovative models and best practices; and develop strategic plans that assure quality care and cost-effective services, aligned with federal, state, and local regulations.
  • Develops, implements, and monitors operational, quality, and service delivery goals and targets through our strategic and operational OKR (Objective Key Results) process.
  • Partners with health care providers outside the organization, in collaboration with the CMO, to achieve optimal participant care and outcomes across the continuum. Develops, oversees, and manages systems and relationships with external providers.
  • Ensures all clinical and nursing policy, safety, protocols, guidelines, and facility requirements are being met.
  • In conjunction with peer senior leaders, leads clinical program team in audit activities, including audit readiness activities. Ensures adherence to required monitoring and surveillance requirements for process and quality improvement initiatives.


Qualifications:

A current California RN License is a must-have


  • Bachelor’s degree in an accredited school of nursing required (BSN)
  • Master’s degree in a healthcare-related field required, or a combination of BSN and relevant experience.
  • 5-10 years experience in a senior leadership role with management experience in clinical environments, preference for ambulatory care, community-based medical clinics, and case management required
  • Lean Six Sigma or equivalent process improvement certification preferred
  • Knowledge of PACE care delivery model and Interdisciplinary team functions preferred
  • Ability to effect change in a highly matrixed organizational structure.
  • Ability to engage and influence individuals and teams to improve performance.
  • Ability to deploy strategy in partnership with administrative and physician leadership
  • Ability to build and maintain effective communication and working relationships with internal and external customers (i.e. clinical team members, primary care providers, participants, caregivers/families, and center leadership teams)
  • Demonstrated knowledge of, the Federal HMO Act, Nurse Practice Act, PACE, and all other local, state, and federal regulations


The salary range for the Senior Director Operational Excellence position at Center For Elders Independence is $ 175,304 - $ 280,470 per year. Salary based on the market for the Senior Director Operational Excellence position as well as experience, skills, abilities, and work history.
