Job Description

The CFSS Stabilization Coordinator is central to the coordination and delivery of services to youth in a continuum of stabilization services. This position works with the participant and their family (as indicated), and/or treatment team to identify needs and create individual action plans intending to help youth stabilize in their current placement (community, foster care or community-based residential programming) thereby preventing placement disruptions and hospitalizations.

The Coordinator meets regularly with Residential and Community Placement staff to assess and design elements of intervention support. This position will also work collaboratively with the Hourly Respite Coordinator as these two services are complimentary in nature. The Coordinator will engage managers at local Emergency Room Departments to create relationships intended to collaboratively work toward stabilizing youth in the community and ultimately divert youth from access Emergency Room services.

The Coordinator is also responsible to oversee the delivery of Nurturing program services for Caregivers of youth receiving respite services and Family Support Partners that will be working with Caregivers identified as in need of servicers using the Caregiver Strain index.

The Coordinator works with State Agency Service Coordinator and the three domains of home, school/work (as indicated) and community to help develop and maintain a supportive web of care. The Coordinator is responsible to working with the Director, Assistant Director and Stabilization Services Program Manager to manage program service delivery, budget, staff training and supervision and other Division Leadership responsibilities. This position sits on the Extended Leadership Team as a middle manager role.

The qualified candidate must demonstrate an understanding of or readiness to learn trauma informed care. This position must demonstrate an understanding of impact of Adverse Childhood Experiences on family systems; substance use or addiction; mental health and trauma; cultural identity; gender identity; high risk behaviors and the impact of collateral trauma and marginalization. Strong relationship-building skills required. Excellent communication and computer skills required. Valid driver’s license, reliable and safe transportation required.

The qualified candidate will have a Bachelor’s or higher degree in Human Services, Counseling, Social Work or closely related field and at last one year of case management experience. Excellent communication and computer skills required. This is a full-time position located in Central Massachusetts and will include community-based travel and implementation. Work schedule must accommodate the needs of the participants and program delivery schedule. Bilingual/Bicultural candidates are encouraged to apply.
