Primary Purpose: To provide and maintain meaningful relationships with family as part of the greater team to teach the family and the young adult how to navigate the adult transition services. Assist the family and the individual with completing the steps to have in place as the individual turns 22 and leaves the entitled educational system to non-entitlement adult services.

Essential Functions
- Receive referrals from the VP of Children & Family Services or IFFS Senior Case Manager and complete assessment on the individual and their family to determine needed appointments.
- Utilize the existing Individual Family Support Plan (IFSP) in conjunction with the Charting the Life Course tool to assist in the development of the Individual Transition Plan (ITP).
- Provide information and referrals to adult benefit programs the family may be eligible for and assist in teaching families how to complete applications and follow up.
- Provide the Supervisor/Senior Case Manager FSC and/or the VP of Children & Family Services, and the DDS Service Agency updates on the status of the families served.
- Research and Arrange workshops and informational sessions on a number of transition topics for parents and provide after workshop support to follow up for completion.
- Meet regularly with the Supervisor/Senior Case Manager FSC and/or the VP of Children & Family Services for supervision.
- Serve as the principle liaison between the Family Support Center, local High Schools, and Day Programs working with the assigned families during this transition period.
- Submit written case notes monthly and a bi-monthly report on each family in the program. Additional reports/plans as requested.
- Offer and facilitate a support group to parents and young adults.
- Attend scheduled Children & Family Support staff meetings.
- Attend collaborative quarterly meetings with the DDS Area Office as requested.
- Monthly billing and any other fiscal related documents as needed.
- Perform other duties as agreed upon with the Supervisor/Senior Case Manager FSC and/or VP of Children & Family Services.
- Minimum of 3 years experience working with individuals with developmental disabilities and their families.
- Strong communication skills and knowledge of community services, and of local, state, and federal benefit programs.
- Ability to teach the family and the young adult how to navigate the adult transition services. A multi-disciplinary team approach to family services is emphasized.
- Minimum of BA degree in social work, psychology, health services, education, or a related field
- Minimum of 1 year of driving experience and a valid drivers license
- Regular and predictable attendance
