We are looking for a dedicated Remote Nurse Case Manager to join our team. In this role, you will be responsible for assessing, diagnosing, and providing treatment to assigned patients remotely.
Responsibilities
Administer nursing care to patients who are ill, injured, or disabled.
Diagnose conditions and develop patient treatment plans.
Monitor and promptly report changes in patient symptoms or behavior.
Collaborate with physicians and specialists to coordinate patient care.
Educate patients on health maintenance and disease prevention strategies.
Facilitate referrals to other healthcare professionals and medical facilities as needed.
Maintain accurate and up-to-date patient medical records.
Offer compassionate advice and emotional support to patients and their families.
Qualifications
Previous experience in nursing.
Familiarity with medical software and equipment.
Ability to establish rapport and build strong relationships with patients.
Strong problem-solving and critical thinking abilities.
Comfortable working in a dynamic and fast-paced environment.
